Your name, Your contact email and phone number. Appointment details. Brief Notes relating to counselling sessions
An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Why I Collect Your Information
Your information is used to respond to your enquiry, to arrange and manage appointments, provide counselling services, to meet legal, ethical and professional obligations and maintain accurate records.
Your personal information is stored securely using password protected electronic systems. I take reasonable steps to protect your information from loss, misuse or unauthorised access.
Records are kept only for as long as necessary to meet professional legal an insurance requirements before being deleted.
Confidentiality
Everything discussed during counselling is treated as confidential. However, Confidentiality may be broken where there is a serious risk of harm to you or another person. Disclosure is required by law. A court orders the release of information. Where possible I will discuss with you first.
Under UK data protection law you have the right to access your personal information. Request correction of inaccurate information. Request deletion where appropriate . Make a complaint to the ICO.
